Comprehensive Evaluations
Psychological and neuropsychological evaluations are comprehensive assessments aimed at understanding the cognitive, emotional, and behavioral factors that may be impacting a child, adolescent, or young adult’s daily functioning. These evaluations are particularly useful for identifying challenges related to learning, attention, memory, emotional regulation, and social skills. By providing a detailed understanding of an individual’s strengths and areas of difficulty, the evaluation helps guide tailored recommendations for therapeutic interventions, academic accommodations, and personal development strategies. The ultimate goal is to offer insight that supports informed decisions for effective treatment and future success.

Dr. Sara Streufert brings a wealth of experience in delivering compassionate, comprehensive care to children, adolescents, and young adults. Specializing in psychological and neuropsychological evaluations, as well as consultative services, she has worked across a variety of clinical settings, addressing complex mental health needs.
Dr. Streufert’s career began at Providence Sacred Heart Medical Center, where she served as a clinical psychologist for over 10 years. During this time, she worked as the psychologist for the Behavioral Emotional Skills Training Program (BEST Program), a hospital-based day treatment program designed to support children ages 7-12 facing significant behavioral and emotional challenges due to trauma and mental health issues. She also provided psychological evaluations and consultative services for adolescents in acute crisis at the Psychiatric Center for Children and Adolescents (PCCA), an inpatient psychiatric program. At the PCCA, Dr. Streufert worked with adolescents experiencing severe, complex mental health conditions such as suicidal ideation, self-harm, psychosis, and severe mood and anxiety disorders. These adolescents often required urgent, specialized care, and Dr. Streufert’s role included assessing, diagnosing, and providing treatment recommendations. She collaborated closely with multidisciplinary teams to help stabilize these vulnerable individuals and ensure they received the necessary care.
Dr. Streufert has also served as an outpatient psychologist at Providence Medical Group, where she specialized in conducting comprehensive psychological and neuropsychological evaluations for children, adolescents, and young adults. Her expertise spans a wide range of conditions, including intellectual delays, attention deficit/hyperactivity disorder (ADHD), autism spectrum disorder, depression, bipolar disorder, anxiety, and learning difficulties. Dr. Streufert has extensive experience assessing children who have experienced complex trauma, adoption, attachment-related concerns, and prenatal substance exposure.
In addition to her clinical practice, Dr. Streufert has provided consultative services to several agencies in Spokane, offering her expertise to a variety of local organizations. She also conducts consultative psychological evaluations for the Division of Disability Determination Services (DDDS), supporting individuals of all ages in need of assistance.
Currently, Dr. Streufert’s private practice focuses on providing specialized, comprehensive psychological and neuropsychological evaluations for children, youth, and young adults through a self-pay model. Her practice is committed to meeting the clinical needs of young people in the community, offering flexibility and avoiding the limitations often imposed by insurance providers.
Dr. Streufert earned her PhD in Counseling Psychology from Western Michigan University and holds both undergraduate and master’s degrees from Gonzaga University. She completed her APA-accredited internship at the Ethan Allen/Lincoln Hills School within the Department of Juvenile Corrections in Irma, WI. Additionally, Dr. Streufert gained valuable experience through doctoral-level externships at the counseling centers at University of Wisconsin, Madison, and Western Michigan University.
With her extensive training and diverse experience, Dr. Streufert is dedicated to helping individuals and families navigate mental health challenges with compassion, professionalism, and a focus on long-term well-being.

Fees

Clinical interview and consultation services are provided in 90-minute appointments for a fee of $350. If you choose to proceed with a comprehensive evaluation at the end of the session, this fee will be applied toward the total cost of your evaluation.

Comprehensive psychological and neuropsychological evaluations are billed at an hourly rate of $250. An additional fee for testing materials and assessment tools may apply, depending on the specific measures used. All costs will be clearly outlined before testing begins.
A comprehensive evaluation typically includes:
A diagnostic interview with the client (and parents or caregivers, if applicable)
A thorough review of relevant records
In-person test administration across one or more sessions
Scoring and interpretation of test data
Integration of findings and report writing
A detailed, written evaluation report
A 60-minute feedback session to review results and recommendations
Because each evaluation is individualized, the number and type of measures used may differ. The total number of hours required—and therefore the final cost—will depend on the unique needs and goals of the assessment.
Scheduling and Payment
To schedule a comprehensive evaluation, a $1,000 deposit is required. This deposit will be applied toward the total cost of the evaluation. The remaining balance can be paid in equal installments at each appointment or in full prior to or on the day of the first appointment.

Currently, Dr. Streufert does not participate with insurance provider networks. While the cost of an evaluation can be a significant consideration for families, it's important to understand that a thorough and accurate assessment can provide meaningful insights and guidance that support an individual’s well-being, development, and daily functioning.
Dr. Streufert can work as an Out-of-Network provider with your insurance company. Many insurance plans cover a significant portion of the fee. In this case, you would pay Dr. Streufert directly at the time of service, and depending on your insurance provider and plan, you may be able to receive reimbursement for much of the cost. To facilitate reimbursement, Dr. Streufert can provide you with a “superbill” that you can submit to your insurance company.

Many clients are eligible for partial reimbursement from their insurance provider for Out-of-Network (OON) services. Coverage for OON services varies by insurance company and individual plan. It is recommended that you contact your insurance provider prior to beginning the evaluation process to better understand your specific benefits.
When speaking with your insurer, consider asking the following questions:
Do I have Out-of-Network coverage?
What are my Out-of-Network benefits?
Do I need a referral or prior authorization to see an Out-of-Network provider?
What is my annual deductible, and what amount has already been met this year?
After I meet my deductible, what percentage or dollar amount of the visit cost will I be reimbursed for? (Note: Be sure to reference the specific CPT codes Dr. Streufert will be billing- see below)
Is there a limit to how many sessions “units” (for testing clients), or dollar amount I can be reimbursed for per year?
Will Telehealth (video) services be reimbursed by my plan?
What is the process to submit claims for reimbursement?
Services are billed using Current Procedural Terminology (CPT) codes, which are 5-digit codes specific to each service. Dr. Streufert can give you a “superbill”, which is the document that has all of the information that the insurance company will need in order to process your reimbursement. Prior to calling your insurance company, clarify with Dr. Streufert which CPT code(s) she will bill, so that you can ask your insurance company specifically.
Common assessment CPT codes:
90791 (intake appointment)
96136 and 96137 (psychological test administration and scoring)
96130 and 96131 (psychological evaluation services)
96132 and 96133 (neuropsychological evaluation services)
Common therapy CPT codes:
90791 (intake appointment)
90832 (individual therapy 25-30 minutes)
90834 (individual therapy 45-50 minutes)
90837 (individual therapy 55-60 minutes)
90846 (parent session without child present)
Telehealth visits are typically designated by adding "-95" to the above codes (e.g., 90834-95 is the CPT code for a 50-minute video therapy session).

Many families choose to use their Flexible Spending Account (FSA) or Health Savings Account (HSA) to cover the cost of medically necessary mental health services, such as comprehensive evaluations, consultation services, and individual or group therapy.
*Note: Standalone IQ testing for school admissions is not typically reimbursed by insurance or by HSA/ FSA accounts due to it being educational in nature rather than medically-necessary. If you plan to use these funds for this purpose, be sure to confirm coverage directly with your insurance provider or plan administrator before scheduling the evaluation.
Limitations of Services
Dr. Streufert is dedicated to supporting a broad range of mental health and assessment needs. However, certain specialized services are currently unavailable, including:
Legal and forensic evaluations
Co-parenting evaluations
Custody evaluations
Court-mandated assessments
